The first thing she did was develop a list of symbols (a legend or key) to identify various activities. R = Run, B = Bike, W = Weights, etc. As you can see, Sue writes in the date of the month in the upper left-hand corner of each square. If you plan to start your training on April 1, 2001 (a Sunday) you would put a 1 in the upper-left-hand corner of the first square underneath the Sunday heading. Then you would write the month - April - down the side of the table as Sue did. Then you can use your symbols to show what activity you did, along with appropriate numbers. 3R would mean that you ran three miles. 1W would mean that you spent an hour working out with weights.
